Census functionaries are to use the document retrieval form to return the various materials and instruments used during enumeration to the supervisor for clearance and payment processing.  The transmittal form is also known as Document Retrieval form - NPC06. There are other available NPC forms for the purpose of census activities in Nigeria. Uses of Census Transmittal Forms Enumerators are expected to return the following materials to their respective supervisors at the end of the census exercise using the transmittal or document retrieval forms. Personal Digital Assistant (PDA) Charger Power Bank Unused stickers Other documents used and unused during enumeration. Note:  Enumerators will be required to filled and signed all necessary forms for clearance. The supervisor will hand over these materials to the field coordinator, who in turn gives it to  the state coordinator for clearance. Don't use the census instruments and materials for personal use.
